This form is to be completed by Old Girls wishing to obtain a copy of records from St Mary’s Anglican Girls’ School’s archives.

St Mary’s will endeavour to assist with all reasonable requests in a timely manner. To assist us in this, we require proof of identity and payment of an administration fee (refer Identification and Payment sections below) as part of this request process.

Western Australian Certificates of Education (WACE) and Western Australian Statements of Student Achievement (WASSA) are not held by St Mary’s. If you wish to obtain a copy of these records, please contact SCSA directly.

• An administration fee of $25 is payable in advance for a Verification of Attendance.

• An administration fee of $35 is payable in advance for Reports, School Certificates, Statement of Achievement/Testimonial and/or Statement of Results.

Submission

If you have opted to collect the records in person, you will receive an email advising when they are available for collection from St Mary’s Senior School Reception. The identification document uploaded with this form will need to be shown when collecting the requested records.
